Puyallup Bliss AFH LLC is a licensed adult family home providing residential care services for seniors, including:

  • Assistance with daily living activities

Eligibility for Services

Our services are available to adults who:

  • Require assistance with daily living activities

  • Need memory care or dementia support

  • Require mental health specialty care

  • Meet Washington State Adult Family Home admission criteria

Admission is subject to assessment, availability, and mutual agreement between the resident (or their authorized representative) and Puyallup Bliss AFH LLC.

Admissions Process

Assessment: All prospective residents must undergo a comprehensive assessment to determine care needs and suitability for our facility.

Agreement: Upon acceptance, residents and/or their authorized representatives must sign an Admission Agreement outlining:

  • Services to be provided

  • Fees and payment terms

  • Rights and responsibilities

  • Duration of stay and termination conditions

Required Documentation: Residents must provide:

  • Medical history and current health information

  • Medication lists and physician orders

  • Insurance information

  • Emergency contact information

  • Legal documentation (power of attorney, advance directives, etc.)

Fees and Payment

Payment Terms:

  • Fees are determined based on level of care required

  • Payment is due monthly in advance

  • We accept Medicare, Medicaid, Private Pay, and Long-term Care Insurance

  • Late payments may result in late fees or service termination

Fee Changes:

  • We reserve the right to adjust fees with 30 days' written notice

  • Fee increases may occur due to changes in care needs or operational costs

Refunds:

  • Refund policies are outlined in the Admission Agreement

  • Advance notice is required for voluntary discharge

Resident Rights and Responsibilities

Resident Rights: Residents have the right to:

  • Receive respectful, dignified, and personalized care

  • Privacy and confidentiality

  • Participate in care planning decisions

  • Voice concerns and file complaints without retaliation

  • Access personal belongings and maintain independence

  • Receive visitors during designated hours

  • Be free from abuse, neglect, and exploitation

Resident Responsibilities: Residents and their representatives are responsible for:

  • Providing accurate health and personal information

  • Following facility rules and policies

  • Treating staff and other residents with respect

  • Timely payment of fees

  • Notifying staff of changes in health status or care needs

Termination of Services

Voluntary Discharge: Residents or their representatives may terminate services with written notice as specified in the Admission Agreement.

Involuntary Discharge: We reserve the right to terminate services if:

  • The resident's care needs exceed our capabilities

  • Payment obligations are not met

  • The resident's behavior poses a danger to themselves or others

  • The resident or family violates facility policies

  • Required by law or regulatory authorities

Involuntary discharge will be conducted in accordance with Washington State regulations and with appropriate notice.

  • Submit inquiries and schedule tours

Prohibited Use: You may not:
